Referral Procedure : What Happens Next
On the agreed date the young person
and his/her belongings will be collected by a 16 Plus
representative at the agreed time and location and transported
to the Placement where they will be met by the Keyworker
who will spend the next couple of hours getting to know
the young person whilst they are out shopping for essential
items for the flat.
The Young person and his/her keyworker
will negotiate and agree days and times for keywork
sessions to take place.
The keyworker will carry out a 6 week
initial assessment covering all areas of the child’s
developmental needs as identified in the Department
Of Health assessment framework paying particular attention
to self care and independent living skills.
The keyworker will produce a care
plan indicating what work will be done with the young
person to meet his/her needs as identified by the initial
assessment.
The young person and keyworker will
work towards meeting the objectives identified in the
care plan and the Social worker will receive a monthly
written progress report which the young person will
have had an opportunity to read and add his/her comments
to before signing.
The Care plan will be reviewed at
12 week intervals.
